Mukhya Mantri Sehat Bima Yojana (MMSY) is Punjab's universal health insurance scheme that provides cashless treatment up to ₹5 lakh per family per year at empanelled hospitals across India.
The scheme covers all residents of Punjab and provides free medical treatment for secondary and tertiary care including hospitalization, surgeries, and daycare procedures.

Registration Process
Step 1: Visit mmsy.in and fill the family declaration form online
Step 2: Generate and download the PDF declaration form
Step 3: Visit nearest CSC/Sewa Kendra or health camp with required documents
Step 4: Complete biometric verification and receive your Sehat Card
Step 5: Start using your card for cashless treatment at empanelled hospitals
Important Note
This online form is just the starting step. After filling the form online and generating the PDF, you must visit a registration center (Common Service Centre / Sewa Kendra / health camp) with your documents to complete the registration and receive your physical Sehat Card.
